Malta House Of Care Eligibility Requirements
Free Primary Health Care in Waterbury, CT
If you are underinsured – and need primary health care in the Greater Waterbury community – then, the Malta House of Care can help.
There are only three criteria to meet for receiving care at Malta Waterbury:
- You must be age 18 or older.
- You may not have insurance of any kind.
- You must bring a photo ID.
By Appointment Only. The Malta Clinic is not an Urgent Care and will only see patients by appointment. If you are a new patient please call the clinic at 203-528-3603 to establish care as a new patient.
Additional services are provided in partnership with the Waterbury Health Access Program (WHAP). WHAP Case Managers can assist Malta Waterbury patients to: (1) enroll in a health insurance program; (2) find a medical home or specialist; (3) or apply for free or low-cost prescription drug programs. WHAP case managers speak English, Spanish and Portuguese.
